I would stick to topical. Clindamycin is great.
OTC benzoyl peroxide & salicylic acid
Anything topical is totally fine, even Retin-A.
Do be careful about getting it ON the kid if you’re using Retin-A; when my daughter was very young she’d go to bed, I’d put it on, leave it on for a few hours, then wash it off before going to bed with her…
From Lactmed
"Topical tretinoin has not been studied during breastfeeding. Because it is poorly absorbed after topical application, it is considered a low risk to the nursing infant.[1][2] Ensure that the infant’s skin does not come into direct contact with the areas of skin that have been treated. Only water-miscible cream or gel products should be applied to the breast because ointments may expose the infant to high levels of mineral paraffins via licking."

But the oral antibiotics aren’t a great idea, and Accutane is definitely a no-go.
Dr. Hale is the author of Medications and Mothers Milk. This is the only book that i trust in regards to how medications the mother takes are affected in the breastmilk. Dr. Hale has created a web page for mothers. The website is wonderful it has lots of useful information but the best part is that it has a toll free number to call to ask about medications. The website is http://www.infantrisk.com/ – take a look at it. The toll free number is: (806)-352-2519
